The advantages and disadvantages of the Delphi method are worth weighing carefully before you commit to it. Like any research approach, it suits some questions better than others, and knowing where its limitations lie is part of using it well.

The Delphi method is a systematic, iterative way to reach consensus among a panel of experts, using rounds of anonymous questionnaires with controlled feedback so that decision-making rests on the genuine convergence of expert opinion. It was developed by the RAND Corporation in the 1950s and is now widely used in healthcare to define best practice, develop clinical guidelines and inform policy, particularly where evidence is limited, ambiguous, or impractical to generate any other way.

Structured communication. The method’s systematic use of surveys and controlled feedback means a conclusion is reached in a disciplined way. Rather than an open-ended discussion that may or may not converge, the process is designed to move a panel towards a clear, defensible consensus that addresses the research question directly.

Unbiased, anonymous input. Participants respond anonymously, which lets them express a genuine view without fear of repercussion and without being swayed by the stated opinions of others. In fields where senior clinicians and prominent researchers sit on the same panel, this is one of the method’s most valuable safeguards.

Equal weight for every opinion. Because responses are anonymous and lengthy live discussion is avoided, every participant’s view carries equal weight. This removes the effect that dominant individuals can have on group dynamics – the risk, in a face-to-face setting, that the most senior or most vocal voice shapes the outcome disproportionately.

Geographical reach. The method is facilitated virtually, so a panel can span countries and continents without the logistical limits of in-person research. That opens access to a genuinely diverse group of experts across specialties, health systems and regions, which strengthens both the credibility and the generalisability of the consensus.

Cost and time efficiency. Because the process is run through secure online tools, it avoids the travel, venue and scheduling costs of convening experts in person, and the controlled-feedback structure keeps it moving rather than stalling in open debate. A robust consensus can be generated in a matter of months rather than years, which matters when a clinical or access decision cannot wait for the trial base to catch up.

The method is not the right instrument for every question, and a credible provider is candid about where it falls short. Each of its main limitations is real, and each is manageable with good study design.

Limited open discussion. Controlled feedback is a strength for reducing bias, but it means participants cannot debate ideas openly or elaborate on their reasoning the way they could in a live forum. For highly polarised questions that need deeper exploration, that constraint matters. It is typically managed by building a small expert steering group into the study design, so the nuance is captured where it counts, in how the statements are framed.

Commitment across multiple rounds. A Delphi study can ask participants to respond to more than one round of questionnaires, sometimes revisiting similar questions. If experts disengage between rounds, that can reduce the number of responses and, potentially, the quality of the result. A well-designed study plan anticipates attrition and manages panel engagement deliberately, rather than leaving it to chance.

Dependence on respondent expertise. The Delphi method is used precisely where there is no absolute answer and expert opinion is the most valuable data available. That places a great deal of weight on the qualification of the panel: if the right experts are not recruited, it is difficult to reach clear or defensible conclusions.
